Reza Salman Roghani, MD

Reza S. Roghani, MD, PhD, is an Assistant Professor of Interventional Spine and Pain Medicine in the Department of Physical Medicine and Rehabilitation at the Johns Hopkins University School of Medicine.

His clinical and academic focus is complex spinal pain, particularly in patients with multiple pain generators, prior spine surgery, altered anatomy, or persistent symptoms despite previous treatment. His approach combines careful diagnostic evaluation with stepwise, image-guided treatment aimed at identifying the main sources of pain and improving function and mobility.

His clinical interests include vertebrogenic pain, advanced radiofrequency procedures and rhizotomy, minimally invasive lumbar decompression, neuromodulation, restorative neurostimulation, and ultrasound-guided peripheral nerve interventions.

His research focuses on defining and measuring spinal pain complexity, improving patient selection and treatment sequencing, and evaluating outcomes after advanced spine interventions. He is developing the proposed Spinal Pain Complexity Index and is involved in building the Johns Hopkins Restorative Spine Registry. He is also developing the textbook Complex Spinal Pain: Advanced Interventional Strategies for Multiple Pain Generators and the Operated Spine.

Dr. Roghani earned his medical degree from Shahid Beheshti University of Medical Sciences and his PhD from Karolinska Institute. He completed a clinical research fellowship in spine and musculoskeletal medicine at Mayo Clinic and a pain medicine fellowship at Johns Hopkins. Before joining the Johns Hopkins faculty, he served as an Associate Professor of Physical Medicine and Rehabilitation and as Director of the Spine and Spinal Cord Medicine Fellowship Program at Shahid Beheshti University of Medical Sciences. He has more than 20 years of experience in spine medicine, research, procedural education, and advanced image-guided interventions.
