Background

Dr. Mathioudakis, an endocrinologist and researcher, has a strong focus on diabetes management and patient care, recognized through awards like the Baltimore Top Doc. He has contributed significantly to diabetes research, authoring over 150 publications and participating in the development of consensus guidelines in the field. His studies have offered valuable insights into using health informatics for diabetes prevention and management.

After graduating from the Johns Hopkins University School of Medicine and completing his training in Internal Medicine and a fellowship in Endocrinology, Diabetes, & Metabolism, Dr. Mathioudakis joined the faculty in the Division of Endocrinology, Diabetes & Metabolism in 2012. He earned a Master of Health Science in clinical investigation in 2015 and was the Clinical Director of the Division of Endocrinology from 2016 to 2021. From 2021 to 2025, he co-directed the Baltimore Metropolitan Diabetes Regional Partnership, focusing on improving access to diabetes prevention and management programs.

As a physician-scientist, Dr. Mathioudakis leads multiple research studies funded by the NIH, private philanthropic organizations (Helmsley Charitable Trust), non-profit organizations (Breakthrough T1D), and industry. His research focuses on the effectiveness of automated diabetes prevention programs and strategies to reduce racial disparities in diabetes technology access. He also collaborates nationally to enhance care for people with type 1 diabetes.

Holding a joint appointment in the Division of Biomedical Informatics and Data Science, Dr. Mathioudakis integrates machine learning and AI in diabetes research. He has served on the national Epic Endocrinology Specialty Steering Board and chaired the Type 2 Diabetes Prognostics Working Group for the Precision Medicine Diabetes Initiative. Committed to education, he frequently lectures on endocrinology, epidemiology, and health informatics.

Nestoras Mathioudakis Lab - Lab Website

  • Our lab is focused on developing and evaluating clinical decision support systems, technology, and mHealth for diabetes prevention and management. We leverage large electronic medical databases and use machine learning algorithms and artificial intelligence to identify patterns in clinical care associated with optimal clinical outcomes. We are interested in understanding the role that advanced diabetes technologies can play in improving health outcomes for patients with diabetes.

Research Summary

Dr. Mathioudakis's research focuses on transforming diabetes care through innovation and technology, guided by the principles of prediction, precision, and prevention. His research is dedicated to developing and evaluating cutting-edge clinical decision support systems and utilizing mobile health (mHealth) solutions to enhance diabetes prevention and management.

A key aspect of Dr. Mathioudakis' work involves the application of machine learning to extensive electronic health record (EHR) datasets. By employing the Observational Health Data Sciences and Informatics (OHDSI) framework, he aims to generate real-world evidence that can predict outcomes and improve decision-making processes for individuals at risk for or currently managing diabetes. This includes investigating AI-driven interventions and conducting a variety of rigorous studies such as retrospective analyses, prospective cohort studies, randomized controlled trials (RCTs), systematic reviews, and meta-analyses.  Additionally, Dr. Mathioudakis addresses healthcare disparities in diabetes care by leveraging informatics tools and advanced technologies to ensure equitable access to effective prevention and management strategies across diverse communities.
